How Can I Become A Patient?

Our patients are the reason Volunteers in Medicine was created. Our patients are the people we see every day in our community. Our patients work hard and are focused on doing their best to provide for their families. We want to help the working uninsured stay healthy and have access to primary care services. 

The facility is not an emergency room or a walk-in clinic, but an appointment only primary care facility to provide preventative medical care to the local working uninsured. Patients will have to meet certain criteria. As part of the Volunteers in Medicine network, we provide care at no cost to our patients. We do not take any form of insurance and do not charge co-pays for our services. If you have a medical emergency please call 911 or go to the nearest emergency room.

Eligibility Requirements

To become a patient of Volusia Volunteers in Medicine you must meet certain criteria. All visits are appointment only, we do not take walk-ins. As part of the Volunteers in Medicine network, we provide care at no cost to our patients. We do not take any form of insurance and do not charge co-pays for our services. If you have a medical emergency please call 911, or visit the nearest emergency room, we do not provide emergency care.  

Eligibility documentation is required by Volusia Volunteers in Medicine staff. To become a patient of our clinic you will have to go through a phone screening. If you are eligible we will inform you of the information that needs to be brought to your appointment.

  • Not Currently Covered By Health Insurance
  • Residence: Must be a resident of Volusia County and possess a permanent street address for at least six months. 
    • Patients must be able to demonstrate permanent residence in Volusia County, FL.
  • Age: Patients must be between the ages of 18-64.
    • No Medicare Patients
  • Employment: Must be employed or employed within past 12 months.
    • Must show current proof of employment or proof of recent employment within the past 1 year.

    • If currently unemployed must provide documentation that the unemployment status occurred within the past 12 months.

  • Income: Patients must be below the 200% Federal Poverty Guidelines

What To Bring To Your Appointment

  • Any Information Discussed During Eligibility Screening
  • Picture ID
  • Previous Medical Related Information (Medication List, Previous Tests/Scans etc.)

Appointments

Patient appointments for medical services can be made by calling us.  Community members who think that they qualify for services at the clinic may contact us by phone or email, to ask about the qualifications and start the application process.
